When you add a new question to your form, you need to choose which type of question you want it to be. Review the content below for a brief description and screenshots of each form field type.
Short Response
Short response form fields allow single line text inputs. Use a short response question when you want a user to enter short text.

Paragraph
Paragraph form fields allow multiple lines of text input. Use a paragraph form field when you want a user to enter longer text, such as descriptions or additional terms.

Dropdown and Multiple Choice
Dropdown and multiple choice form questions allow you to populate contract fields from a set of well-defined possible values.
The only difference between dropdown and multiple choice questions is how they are displayed on the form: dropdown questions are answered by selecting one or more options from a dropdown menu, whereas multiple choice questions are answered by selecting one or more radio buttons. See the examples below.

Email form fields allow you to collect email addresses from your users. Your users should not submit a form if their email is invalid.

Address
Address form fields allow you to collect addresses from your users. Your users cannot submit a form if their address is missing a required field.

Number
Number form fields allow you to collect numerical values from your users. Non-numeric values are not allowed. You can build conditions based on the number value entered.

Numbers to Words Filter
After you set up a number form field question, you can adjust how that number appears in your document.
- Click on the Workflow Designer tab and select the workflow configuration you want to modify.
- Click on the Document tab.
- Find the number field tag and right-click on it.
- Click Format and select your formatting preference. The field is automatically converted to your selected format in your launch form. For example, 100 can be automatically updated from “100” to “100th”.
Monetary
Monetary form fields allow you to collect monetary amounts from your users. Your users can select their currency type and enter their value. You can build conditions based on the currency and monetary values entered.

Date
Date form fields allow you to collect dates from your users. Your users can type in a date or select a date from a calendar. You can build conditions based on the date values entered.

File Attachment
File attachment form fields allow you to collect additional files from your users. Your users’ files are uploaded to cloud storage at the path configured at Archive > File Management > File Paths > Uploads. The files are also attached to the contract record when the workflow is archived.
The following file types can be uploaded for file attachments that are not included in the signature packet:

Yes/No
Yes/No questions are multiple choice form field questions with preset yes and no values. These values are not editable.
Yes/No form field questions are searchable as such in Repository. The record type for these questions is Yes/No and they can be added as additional record properties to show up in Archive only. This searchability is the key difference between selecting a yes/no form field instead of creating a Yes/No multiple choice form field question.

Related Record
Related records enable you to connect workflows to one another. You cannot connect two active workflows together. You can only select a completed workflow record.

Help Text
Help text form fields allow you to provide additional information on launch forms, counterparty forms, and internal forms. You can use this form field to link to resources, checklists, descriptions of parallel processes, and advise users as to what happens after they submit.
